I hereby give my permission and authorize representatives of Reach Out and Read Kansas City to utilize any volunteer project photos taken of me in print or electronic promotional materials and to investigate any or all of the statements I have made in this application. As part of my application I consent to allowing ROR to obtain a criminal background investigation, if ROR decides to do so. In the event ROR receives a report that adversely affects my acceptance as a volunteer, I will be provided a copy of that report. I will have 24 hours to provide information if I believe the report is incorrect.
I understand that this application does not guarantee a volunteer placement through ROR.
